Interested in signing up for a WageWorks® HSA but not sure if you qualify? Take this short quiz to find out.
Are you covered under a high-deductible health plan (HDHP)?
Your answers to questions 2 and 3 will help you determine if you are covered by an HSA eligible health plan.
Does your health insurance plan have an annual deductible of not less than $1,350 per individual and $2,700 for family coverage?*
Check your health plan materials or contact your health plan provider and then return to take this test.
Does your health insurance plan have an annual out-of-pocket maximum of no more than $6,650 for individual coverage and $13,300 for family coverage?*
Are you covered by Medicare (Part A or Part B)?
You may want to check with those who care for and about you (including your spouse, if you are married) to see if you are covered under a Medicare plan.
Does another person claim you as a tax dependent on their tax return form?
If you file your own tax return form each year, you are probably not claimed as a tax dependent by another person. If you do not file your own tax return form each year, you may be claimed as a tax dependent by another person. Check with those who care for and about you if you are not sure .
Are you covered by a supplemental or cancer plan, discount card or discount price program, employee assistance program (EAP), wellness or disease management program, or insurance for a specified disease or illness?
Your enrollment in any of the listed plans does not affect your qualifications for an HSA. If you are not sure if you are enrolled in any of these plans, then select No.
Are you covered by another health plan (such as your spouse's plan)-other than those listed in question #6 that is not an HDHP?
If you are married, you should check with your spouse to see if you are covered under a health plan through your spouse's employer and whether or not that plan is a high-deductible health plan.
Are you covered by any other healthcare benefit account, such as a Healthcare Flexible Spending Account (FSA) or an employer-funded Health Reimbursement Arrangement (HRA), that covers medical expenses in addition to dental and vision expenses?
Check the list of eligible expenses that came with the materials for that plan or ask your plan administrator and then return to take this test.
